Job Description
Job Description: Keyworker to manage caseload for Older Adults with varying mental health issues, co-morbidities and complex presentations. Carrying out assessments, reviews, interventions and completing discharges alongside relevant documentations to support.
Proficient in use of RiO documentations.
Administering Depot injections.
Completing multi-disciplinary reviews in the community and inpatients on Mental Health Act sections.
Liaising with relevant members of the multi-disciplinary team and external agencies to support patient care.
